February Issue of AHRQ Web M&M Discusses Dangers of Failed Contact with Patients

The February issue of AHRQ Web M&M features a Spotlight Case that describes a case involving a young woman who presented to the emergency department with heavy vaginal bleeding, for which she received treatment and was discharged the following day. The patient also tested positive for chlamydia, but had left the hospital by the time the results were complete. Repeated follow-up attempts were made to contact the patient using the contact number in the electronic health record, but were unsuccessful. After receiving a letter from the hospital, the patient returned, received intravenous antibiotics, and was expected to make a full recovery. A commentary, written David W. Shapiro, M.D., J.D., acknowledges the lack of evidence-based measures to decrease the incidence of failed contacts, but offers suggestions for clinicians and health care organizations to reduce patient harm. The Perspectives on Safety section features an interview with Beverley H. Johnson, president of the Institute for Patient- and Family-Centered Care.
