Now Available for Review and Comment: Common Formats for Surveillance–Hospital

The Agency for Healthcare Research and Quality (AHRQ) has released Common Formats for Surveillance—Hospital – which includes modules entitled Generic Adverse Event Information, Blood or Blood Product, Delivery-Maternal, Delivery-Neonatal, Device or Medical/Surgical Supply Including Health Information Technology (HIT), Fall, Medications, Pressure Ulcer, Readmissions, Surgery or Anesthesia, Venous Thromboembolism, and Other Outcomes of Interest.

Until now, Common Formats have been designed to support only traditional event reporting.  Common Formats for Surveillance—Hospital are designed to provide, through retrospective review of medical records, information that is complementary to that derived from event reporting systems. These formats will facilitate improved detection of events and calculation of adverse event rates in populations reviewed.

The Agency has contracted with the National Quality Forum (NQF) to assist with gathering and analyzing feedback. To view Common Formats for Surveillance—Hospital and post comments through the NQF tool, please go to Patient Safety Organization (PSO) Privacy Protection Center (PPC) Web site at www.psoppc.org.

AHRQ appreciates the time and effort individuals invest in providing comments. All feedback received will be reviewed and considered to guide the development of a revised version.
