ISMP Survey on Pediatric Medication Practices.

Children are particularly susceptible to medication errors. This survey seeks input from health care workers who prescribe, administer, and dispense medications for pediatric patients to track types of errors and identify weaknesses in medication processes. The deadline for submitting the survey results is April 10, 2015.
