The concept of preventability can be difficult to apply to avoidable hospital mortality. Examining the proposed use of hospital-acquired harm as a ranking mechanism in the United Kingdom, this commentary outlines the weaknesses of this measure, such as how resource constraints can limit appropriate case review, and recommends potential solutions.
