Deaths following prehospital safety incidents: an analysis of a national database.

Safety problems in prehospital care can lead to adverse events. This study examined patient deaths related to ambulance safety incidents and found that the majority of problems were due to dispatch issues, poor clinical decisions, and equipment failures.
