Medication safety systems and the important role of pharmacists.
Preventing adverse drug events is a major priority for accrediting and regulatory agencies. This review describes a framework for medication safety systems, including design considerationsto integrate safety across the medication use process and unique roles for clinical pharmacists. Elements of the framework address risk awareness, barriers to error reporting, and the need to utilize performance improvement methods.
