Reminder: CAHPS^® Health Plan Survey Database Submission June 6 - 24

CAHPS 5.0/5.0H Health Plan Survey users who administered the survey between October 2015 - June 2016 can submit their data to the CAHPS Database June 6 - 24.

Data collected from surveys administered October 2015 - June 2016 will be accepted for the following survey versions:

• Adult Medicaid
• Child Medicaid (or CHIP) without the Children with Chronic Conditions Item Set
• Child Medicaid (or CHIP) with the Children with Chronic Conditions Item Set

The submission requirements, Data File Specifications, and Data Use Agreement (DUA) are available at:https://cahpsdatabase.ahrq.gov/HPDSS/login.aspx. 

All required items must be submitted no later than June 24.
