According to the Institute for Patient- and Family-Centered Care (IPFCC), “patient- and family-centered care” is an approach to the planning, delivery, and evaluation of health care that is grounded in mutually beneficial partnerships among health care providers, patients, and families. A growing body of research indicates that partnering with patients can lead to measurable improvements in safety and quality. Health care organizations are working with patients in various ways to implement innovative strategies to deliver patient- and family-centered care.
The featured Innovations describe a medical center whose patient and family advisors spearheaded a redesign of the bedside change-of-shift reporting process, a hospital that established a patient and family advisor rounding program to improve quality of care, and an outpatient rehabilitation center that revamped its patient education program for spinal cord injury patients to incorporate a variety of patient-centered, peer-based approaches.
The featured QualityTools include a range of resources and tools designed to support efforts to partner with patients and families to improve health care quality.
This issue also includes a featured Perspective, Convening a Learning Community To Advance Patient- and Family-Centered Care in Hospitals, based on an interview with the Innovations Exchange PFCC Learning Community's innovator and expert faculty. Representatives of 11 Florida hospitals worked collaboratively to adopt and implement patient- and family-centered care strategies published in the Innovations Exchange.
