The problem with medication reconciliation.

Medication reconciliation has demonstrated safety improvement in both inpatient andambulatory settings. This commentary discusses barriers to reliably implementing medication reconciliation and attributes those challenges to the complexity of health care delivery and the costs involved in developing and sustaining a working process.
