Improving patient safety by practicing in a just culture.

The importance of just culture is widely acknowledged throughout health care, but it remains difficult to implement and assess. This commentary discusses human factors that contribute to blame for errors and suggests roles for both frontline nurses and managers to instill elements of just culture in the practice environment.
