Pharmacist-led admission medication reconciliation before and after the implementation of an electronic medication management system.
Implementation of an electronic medication management system at an Australian hospital was associated with more timely performance of medication reconciliation at admission. High-risk patients—older patients and those taking more than five prescription medications—were also more likely to receive formal medication reconciliation after implementation of the electronic system.
