A new infographic from AHRQ’s EvidenceNOW initiative shows the diversity in the small and medium-sized primary care practices it serves.  In 2015, AHRQ launched EvidenceNOW: Advancing Heart Health in Primary Care to help practices across the country use the latest evidence to improve the heart health of millions of Americans.  The initiative set out to work with some of the Nation’s smallest primary care practices that often lack the resources to focus on advanced quality improvement.  AHRQ and EvidenceNOW’s 7 regional cooperatives are partnering with a diverse group of over 1500 small primary care practices. More than 1 in 3 of these practices is located in a medically underserved area.  Over 75% of the participating practices have fewer than 5 physicians and nurse practitioners, with a third of them being solo practices. Almost 20% of the practices are located in rural communities.  Over 40% of the patients served by the typical practice participating in EvidenceNOW have Medicaid or are uninsured.  View the infographic here.
