SOPS Medical Office Database

AHRQ has established the Medical Office Survey on Patient Safety Culture Database as a central repository for survey data from medical offices that have administered the AHRQ patient safety culture survey instrument and choose to submit their survey data to the Medical Office SOPS database. The database serves as an important resource for patient safety culture improvement.

Current Database Report

2018 Medical Office Database Report (Executive Summary [PDF file – 1.3 MB]; Report [PDF file – 3.6 MB]; Appendixes [PDF file – 2.5 MB])—The latest edition of the AHRQ Medical Office Survey on Patient Safety Culture User Database Report presents data from 2,437 U.S. medical offices. The report consists of a narrative description of the findings and two appendixes, presenting data by medical office characteristics (number of providers, single specialty vs. multispecialty, specialty, primary care specialty, ownership, and region) and respondent characteristics (staff position and tenure in medical office). The report contains data voluntarily submitted by participating medical offices and is not representative of all U.S. medical offices.

2016 Medical Office SOPS Database Report (Report Part 1 [PDF file – 1.58 MB]; Part 2 [PDF file - 1.6 MB])—Based on data from1,528 medical offices, the 2016 user database report provides a narrative description of the findings and two appendixes, presenting data by medical office characteristics and respondent characteristics for the participating medical offices. The report contains data voluntarily submitted by participating medical offices and is not representative of all U.S. medical offices.
2014 Medical Office SOPS Database Report (Part 1 [PDF file  – 2.8 MB]; Part 2 [PDF file  – 1.3 MB])—The AHRQ Medical Office Survey on Patient Safety Culture User Database Report presents data from 27,103 staff within 935 U.S. medical offices. The full report contains detailed data for various medical office characteristics (number of providers, specialty, ownership, and region) and staff position. The report contains data voluntarily submitted by participating medical offices and is not representative of all U.S. medical offices.
A database report was also produced in 2012.
